Tennis elbow, medically known as lateral epicondylitis, is a common condition that affects many individuals, not just tennis players. While tennis elbow is commonly referred to as an “itis” (indicating inflammation), recent research suggests that it is primarily a degenerative condition called tendinosis. As physical therapists, we are going shed light on the nature of tennis elbow and discuss the role of tendinosis in its development and treatment.

Tendinosis is a degenerative condition of the tendon that occurs as a result of repetitive micro-trauma or overuse. Unlike tendinitis, which implies inflammation, tendinosis involves changes in the tendon’s structure without significant signs of acute inflammation. In the case of tennis elbow, tendinosis primarily affects the extensor tendons that attach to the lateral epicondyle of the elbow. The repetitive stress on these tendons leads to collagen breakdown and the formation of abnormal tissue.

Tennis elbow can develop from various activities that involve repetitive wrist extension and gripping motions. While the condition is commonly associated with tennis, it can also occur due to other activities such as painting, gardening, typing, or using hand tools. Factors that contribute to the development of tennis elbow include:

1. Repetitive Movements: Engaging in repetitive activities that strain the extensor tendons, such as swinging a tennis racket or using a screwdriver, can lead to tendinosis over time.

2. Poor Technique: Incorrect form or improper technique during sports or physical activities can increase the strain on the tendons, making them more susceptible to degeneration.

3. Overuse and Fatigue: Overuse of the forearm muscles without sufficient rest and recovery can lead to cumulative micro-trauma, ultimately resulting in tendinosis.

4. Age and Gender: Tennis elbow tends to be more prevalent in individuals between the ages of 30 and 50. Additionally, men and women are equally affected, although men typically experience more severe symptoms.

The management of tennis elbow, specifically tendinosis, focuses on promoting tendon healing, reducing pain, and restoring functional strength. Various strategies are used to address these goals:

1. Manual Therapy: Hands-on techniques, such as soft tissue mobilization and myofascial release, can help improve tissue quality, increase blood flow, and promote healing in the affected tendons.

2. Exercise and Stretching: Specific exercises are designed to gradually load and strengthen the extensor tendons, improving their resilience and promoting tissue remodeling. Stretching exercises for the forearm muscles can also aid in reducing muscle tension and promoting flexibility.

3. Activity Modification: Identifying and modifying activities that exacerbate symptoms is crucial in managing tennis elbow. Adjusting technique, using ergonomic tools, and implementing proper body mechanics can help reduce strain on the tendons.

4. Brace or Splinting: In some cases, a counterforce brace or splint may be recommended to offload the affected tendons and provide temporary relief during activities that aggravate symptoms.

5. Education and Self-Management: Providing education on proper ergonomics, posture, and self-care techniques is vital for long-term management. Teaching individuals how to modify their activities, use appropriate equipment, and incorporate rest and recovery strategies can help prevent recurrence.
